Two Currencies

  • Cash — earned by completing shifts and treating patients. Used for most items and free classes.

Coffee

The most versatile item in the shop. Primarily used for sanity recovery between patients, but has the critical secondary use of neutralizing Head Banger enemies — offering coffee is the only correct response to a Head Banger.

Effect: Restores sanity. Calms the Head Banger enemy.

Chocolate

A straightforward sanity recovery item. More effective than coffee for pure sanity restoration but lacks the Head Banger utility. Good to stock when sanity pressure is high.

Speed Soda

Gives a temporary speed boost. Useful for ambulance response, escaping dangerous situations, or quickly moving between patient rooms during high-pressure moments.

Teddy Bear

A seemingly innocent item with practical value. Placing a teddy bear near anxious patients calms them and reduces the chance of stress-triggered events. Subtle but effective over long shifts.

Effect: Comforts distressed patients. Reduces patient anxiety and prevents some negative events.

Gun (20 rounds)

The most important weapon in the game. This is the ONLY tool that can kill a Skinwalker — no other item or action works. Comes with 20 rounds. Do NOT waste shots on other enemies — most are immune or multiply when shot.

Effect: Lethal weapon. The only way to eliminate a Skinwalker.

Taser (3 uses)

A non-lethal alternative to the gun. Stuns enemies temporarily, giving you time to escape or reposition. Does NOT kill Skinwalkers — only the gun works for that. Best used on Stalkers and roaming threats to create distance.

Effect: Non-lethal stun. Temporarily disables most enemies. 3 uses.

upgradeHigh (Cash) — per-run upgradeHigh — one of the best per-run upgrades. Buy early in shifts where you can afford it.

Auto-Give Medicine

A quality-of-life upgrade that automates basic medicine distribution. Doesn't replace treatment but handles the repetitive medicine-giving step, letting you focus on anomaly detection and event response.

Effect: Automatically administers basic medicine to patients without manual input.

Sanity System Explained

Sanity is your mental health meter. It drains continuously under pressure and drops sharply from traumatic events. Low sanity impairs your vision, judgment, and reaction time — making it harder to spot anomalies when you need to most.

What drains sanity: Witnessing deaths, failed events, prolonged anomaly exposure, staring at Mass of Eyes, Skinwalker encounters.

What restores sanity: Coffee (light restore), Chocolate (moderate restore), completing shifts successfully, Psychologist class passive bonus.

Item FAQ

What should I buy first as a beginner?

Coffee is the highest-priority item — it restores sanity and neutralizes Head Bangers at the admission window. After Coffee: save for the Gun (essential for Skinwalkers from Shift 3+), then Auto-Give Medicine (best per-run upgrade for treatment efficiency). Chocolate is a good backup sanity item if Coffee is not in stock.

How does the Sanity system work?

Sanity drains from witnessing disturbing events, making wrong treatment decisions, prolonged exposure to anomalies and enemies, and failing events. When sanity drops below half, your vision distorts and your judgment becomes impaired — making further mistakes more likely. Coffee and Chocolate restore sanity. The Psychologist class provides passive sanity resistance.

When does the shop unlock?

The Supplies Shop unlocks after your first few shifts (typically after Shift 2). It refreshes its inventory between shifts and you can reroll the available items for a fee. Not all items are available every shift — if a high-priority item like the Gun appears, buy it immediately rather than waiting.

Gun vs Taser — which is better?

The Gun is the higher priority because Skinwalkers (Shift 3+) must be shot to eliminate — a Taser will not kill them. The Taser is situationally useful for stunning roaming enemies to create distance, but it does not kill. Buy the Gun first, then consider the Taser as a secondary tool for higher shifts.

Is the Teddy Bear worth buying?

The Teddy Bear is a niche item — it provides a small sanity buffer when held but takes up an inventory slot that could hold Coffee or a weapon. It is most useful for Psychologist-class players on long shifts where passive sanity resistance stacks with the bear's buff. For most players, Coffee + Gun is a better loadout.

How much Cash do items cost?

Prices scale roughly by priority: Coffee is cheapest (~50-100 Cash), Chocolate and Speed Soda are mid-range, the Gun and Taser are expensive (~300-500 Cash), and Auto-Give Medicine is the most expensive upgrade (~500+ Cash). Save aggressively for the Gun and Auto-Give Medicine — they are one-time purchases that permanently improve your runs.
